Required Documents

A signed terms and conditions form is required in order to begin services. A physical copy will be provided during the client’s first appointment. If the client is unable to be present during their first appointment a downloadable version  is available below. This gives the client the option to download, print, sign, and leave the terms and conditions form in a previously discussed secure location for the scoopers to collect before services begin.
